1. Family Eye Card Privilege Card offer is open to any person/family who has had or wants to avail of the clinical services of Sunetra Family Eye Care centre irrespective of nationality, caste, creed or religion.

2. The person in whose name the initial purchase of the card is made, to be known as the "Primary Member", must be at least 18 years of age.

3. All payments pertaining to the privilege card may be made by cash/crossed DD only in favour of "Calcutta Eye Research Foundation" payable at Kolkata. Payments by credit card/cheque shall not be accepted.

4. This privilege card, offer may not be clubbed with any other offer available/declared at Sunetra Family Eye Care Centre during the period of the offer in question. The defined benefit or offers cannot be exchanged for cash or any other service. Also they cannot be carried over.

5. Free Eye Check-up in a year can be availed only once by a card-holder and on a single day only, irrespective of category of check-up availed. Master Eye check-up & Computer Vision Syndrome Check-up are not included for the free offer.

6. All professional services at Sunetra Family Eye Care Centre related to the offer shall be available by prior appointment only subject to availability of doctors/paramedics and related logistics.

7. The privilege card benefits shall not be available without production of the relevant card by the card-holders. Use of a privilege card bearing a member name shall be non-transferable. Supporting validation by photo ID may be necessary.

8. The Management of Sunetra Family Eye Care Centre reserves the right to extend/withdraw the offer at any time without prior notice or explanation.

9. Regarding interpretation and/or clarification of the terms & conditions of the offer, decision of the management of Sunetra Family Eye Care Centre shall be binding.

10. All services at Sunetra Family Eye Care Centre shall be available at all times even without the offer. Legal disputes, if any, related to the offer, shall be settled within the jurisdiction of the city of Kolkata.
